Transforming Diabetes Care:

Abstract

Managing Type 2 diabetes mellitus (T2DM) in elderly populations requires an individualized, holistic approach that factors in frailty, cognitive status, and comorbidities. The primary clinical objective is maintaining stable glycemic control while strictly avoiding hypoglycemia, reducing polypharmacy burdens, and improving overall quality of life. While recent pharmacological milestones have introduced newer classes of anti-diabetic drugs with established cardiorenal benefits, their specific impact on the very elderly remains understudied and requires cautious initiation. Metformin remains a key first-line treatment when renal function allows, while DPP-4 inhibitors offer a safe, well-tolerated second-line option, particularly for frail individuals.

Redefining Early Alzheimer’s Disease Management in Primary Care

Abstract

Addressing Hong Kong’s escalating cognitive decline burden—where 73.7% of adults aged 60 and above exhibit cognitive impairment and 480,000 live with Mild Cognitive Impairment (MCI)—demands a proactive paradigm shift in primary care practice. This lecture explores how high-precision blood-based biomarkers, specifically plasma pTau-217, now enable early biological identification of Alzheimer’s pathology in routine primary care settings.

Targeting synaptic membrane loss as the principal driver of early cognitive decline, the presentation evaluates the therapeutic mechanism and clinical trial data for Fortasyn Connect. By engaging the Kennedy and PEMT pathways, this daily multi-nutrient formulation supports structural neuronal repair. Results from the landmark LipiDiDiet RCT demonstrate a 76% reduction in memory decline and up to 10.5 months of preserved functional independence. Comparative analysis reveals that Fortasyn Connect matches the clinical effect sizes of standard cholinesterase inhibitors (d = 0.20) without drug toxicity or side-effect burden.

Finally, the session establishes an actionable primary care framework—combining early biomarker screening, immediate synaptic nutritional intervention, and multimodal lifestyle management (FINGERS model) to delay disease progression.

Early Intervention for Patients

Early intervention in psoriasis is essential to reduce symptom burden, prevent disease progression, and improve long-term patient outcomes. This lecture will review the clinical rationale for prompt diagnosis and timely treatment initiation in patients with psoriasis, with emphasis on minimizing inflammatory burden, preserving quality of life, and reducing the risk of comorbidities. We will discuss practical approaches to identifying candidates for early intervention, selecting appropriate therapies, and monitoring response in routine practice. The session aims to support clinicians in integrating early, proactive management strategies into care pathways to optimize outcomes for patients with psoriasis.

Unravelling Strategies across
Cardiorenal Management with SGLT2i

Abstract

Cardiovascular, kidney, and metabolic (CKM) health are tightly interconnected; chronic kidney disease (CKD) amplifies cardiovascular and heart failure (HF) risk, while HF accelerates kidney decline. Risk assessment is evolving beyond traditional atherosclerotic models, with tools like the PREVENT equation integrating CKM domains. Early detection using dual assessment of kidney function and damage—eGFR and urine albumin–creatinine ratio (UACR)—is critical, as albuminuria often precedes eGFR decline.

Sodium-glucose cotransporter-2 inhibitors (SGLT2i) deliver organ protection beyond glycaemia across diabetes status. Latest publications suggest SGLT2is reduce sustained eGFR decline, end-stage kidney disease, and renal death in CKD with increasing evidence in lower uACR use. SGLT2is also reduce cardiovascular death or HF hospitalization and rapidly improve symptoms and health status across LVEF in Heart Failure patients.

聯盟為國家及港府推動體重管理

世界衞生組織指出，肥胖是因脂肪過量沉積而損害健康的一種慢性疾病，並非單純的外觀議題，而是關乎全身性風險的醫療健康問題。數據顯示，香港及中國內地有超過一半成年人屬於超重或肥胖，數字更持續上升，每年為醫療系統及社會經濟帶來沉重負擔。

香港中華醫學會會長兼呼吸系統科專科陳真光醫生表示，為應對日益嚴重的超重及肥胖問題，國家衞生健康委於2024年6月聯合16個政府部門，啟動「三年體重管理年」（2024-2026）。這一全國性公共健康項目旨在透過推廣文明健康的生活方式，應對中國居民超重和肥胖問題日益嚴重的公共健康挑戰，並提出以「預防為主、早期介入及社區參與」為重點，全國一同展開體重管理工程。

與國家策略相呼應，香港特區政府亦早前推出了首份《體重管理行動計劃》，響應國家「體重管理年」的部署和世界衞生組織遏制肥胖症加速計劃全球框架。行動計劃提出5項方針和15項具體目標，推動全港體重管理。行動計劃為期三年，按年度訂立不同主題，包括提高意識、積極改變、以及持續成為生活的一部分。

肥胖與逾224種疾病相關

肥胖不僅是外觀上的困擾，更是涉及全身性影響的慢性疾病。有研究指出，肥胖至少與224種疾病相關，涵蓋心腦血管疾病、「三高」、脂肪肝及多種痛症等；除身體風險外，約60%肥胖患者的精神及情緒健康亦受到負面影響。減重能帶來顯著的健康益處。有研究指出，若三級肥胖人士（BMI ≥ 40公斤/米²）能減重約13%，便可顯著降低多種慢性疾病風險，包括二型糖尿病及睡眠窒息症風險可降低近四成，高血壓、高血脂及哮喘風險則可降低近兩成。

「多運動、少進食」未必長期有效

常用的減重方式包括控制飲食、增加運動、藥物治療及減重手術，然而單靠「多運動、少進食」未必能長期奏效。陳醫生解釋，飲食控制及運動固然能令體重下降，但身體會逐漸出現代謝適應——體內的飽腹荷爾蒙及能量消耗下降，饑餓荷爾蒙則上升，令減重成效遞減；一旦恢復以往的飲食模式，體重有可能反彈。有研究指出，短期內只有約兩成患者能達到並維持10%或以上的體重下降；長期觀察更顯示，超過八成患者會於5年內反彈至原有體重水平。

幫助降低食慾；手術方面，胃旁路手術、袖狀胃切除手術及內視鏡減肥療法等，則透過限制胃容量及減少食物吸收達致減重效果。近年備受關注的「減肥針」，即GLP-1 受體促效劑／GIP+GLP-1雙受體促效劑，屬皮下注射的處方藥物，其機制包括提升飽腹感、延緩胃排空、降低食慾與熱量攝取，以及促進脂肪分解。

Disease Modification vs. Disease Management:
Dual IL-4/IL-13 Inhibition Refining Goals in Pediatric Atopic Dermatitis

Atopic dermatitis is a systemic Type 2 inflammatory disease driven by IL 4 and IL 13, causing barrier dysfunction and immune dysregulation. Emerging pediatric data suggest early dual IL 4/IL 13 inhibition may restore barrier integrity, reduce sensitization, reduce Staphylococcus Aureus colonization and potentially modify disease trajectory that includes preventing comorbidities like atopic march and growth impairment.

Meeting Outcome

Hong Kong needs a coherent prevention pathway embedded within chronic-disease management. The CKM framework offers a practical starting point because it brings together the patients most likely to experience both infection and its downstream consequences. By combining risk-based clinical assessment, brief primary care interventions, clear access pathways, patient-group partnerships and carefully evaluated financing, infection prevention can move from an optional conversation to a measurable component of high-quality CKM care. A conjoint paper is in the works to sammarise the literature and the key messages.

VESALIUS CV and the Future of Atherosclerosis Prevention:
A Paradigm Shift Toward Earlier Action

Abstract

Dyslipidemia is the most important modifiable factor that accumulates with time. The LDL blood level is lower and the earlier the better . Lowering your LDL early in life prevents the buildup of fatty plaques (atherosclerosis), resulting in up to a three-fold greater reduction in heart disease risk compared to starting treatments later in life. The reduction in MACE seen in Vesalius CV trial with achieved LDL-C of 45mg/dL in Evolocumab arm , support intensive LDL-C lowering to this level even in patients without prior major CV event. Evolocumab is the only PCSK9 inhibitor to date with proven CV outcome in a wide range of patients, including primary and secondary prevention, as well as the use in the setting of ACS.

Dr Mohamed AbdelZaher AbdAllah Ibrahim is a senior Consultant Interventional Cardiologist with more than 20 years of clinical, academic, and interventional experience in the field of cardiovascular medicine. He currently serves as Consultant Interventional Cardiology at King Fahd Military Medical Complex in Dhahran, Saudi Arabia, and is a Lecturer of Cardiology at the Faculty of Medicine, Ain Shams University, Egypt, a position he has held since 2008.

Dr AbdelZaher obtained his MB BCh, Master’s degree, and MD in Cardiology from Ain Shams University with excellent academic standing. He is internationally certified, having passed the American National Board of Echocardiography (ASCeXAM) and the European Association of Percutaneous Cardiovascular Interventions (EAPCI) certification.
